Fostering a Respectful Workplace

Our mission is to collaborate with University departments and organizations, through policy and procedure, to:

  • Educate all University members on how to prevent, identify and report discrimination, harassment, retaliation, and gender-based violence.
  • Monitor complaints of discrimination, harassment, retaliation, and gender-based violence, conduct a fair and equitable investigation of each complaint, and use information from monitoring and investigations to inform prevention education.
  • Connect those affected by discrimination, harassment, retaliation, and gender-based violence with campus and community resources that will allow them to move forward.

How to Report

This complaint form should be used to report alleged violations of CSU鈥檚 Policy Prohibiting Discrimination, Harassment, Sexual Misconduct, Sexual Exploitation, Domestic Violence, Dating Violence, Stalking, and Retaliation. 

Instructions: Please fill in all of the information requested on the form as thoroughly as possible and attach additional pages to the form, if necessary.

The campus Director of Title IX and Civil Rights Officer is the designated administrator to receive reports of Sex Discrimination, Sexual Harassment, Sexual Misconduct, Sexual Exploitation, Dating Violence, Domestic Violence, Stalking, and associated Retaliation. 

If you are in immediate danger or require medical attention, please contact the police or emergency medical services at 911. 

 

Policies:

It is the CSU鈥檚 policy to provide equal opportunity for every person regardless of the person鈥檚 protected status. Protected statuses include age, disability, gender, genetic information, gender identity or expression, nationality, marital status, race or ethnicity, religion, sexual orientation, and veteran or military status.

CSU Systemwide policy prohibits discrimination, harassment and retaliation, sexual misconduct, sexual exploitation, dating and domestic violence, and stalking against students, employees, and third parties. The policy also includes the procedures for addressing complaints made against students and complaints made against employees and third parties. An applicant or third party may contact the Human Resources Office or the Equal Opportunity Office at the campus or Chancellor鈥檚 Office to file a complaint related to the complainant鈥檚 protected status.
